  3717-1-04.4 Equipment, Utensils, and Linens: Maintenance and   Operation
   
  VIOLATIONS No change in conditions of the walk-in cooler. The issues are: 1) The back wall partitioned off with sheet of
  plastic separating an unused portion of the cooler. This wall or portion of the cooler must be enclosed with a wall surface
  that is smooth, cleanable. 2) Glass front doors panels to the walk-in cooler are broken and have been replaced with cardboard.
  They must repaired / replaced.
   
  (A)      Equipment - good repair and proper adjustment.
   
        (1) Equipment shall be maintained in a state of repair and condition that meets the requirements specified under
       rule 3717-1-04 of the Administrative Code and rule 3717-1-04.1 of the Administrative Code.
   
        (2) Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and
        adjusted in accordance with manufacturer's specifications.

  3717-1-05.1 Water, Plumbing, and Waste:  Plumbing System
   
  VIOLATION No changes made in this area. 1)There is no hot or cold running water available in the restroom for handwashing or
  toilet flushing due to poor repair of the water lines to the restroom. Make hot and cold running water available to the
  employees restroom. As of now, jugs of water are brought in and are used for toilet flushing and handwashing. 2) The floor
  drains back up with standing water. Repair the plumbing for these drains.
   
   
  (S)      Plumbing system - maintained in good repair.
   
       A plumbing system shall be:
   
       (1)      Repaired according to the Ohio  building code; and
   
       (2)      Maintained in good repair.

  VIOLATIONS Some improvement made in this area.1) There is still accumulations of plastic, cardboard, paper, plastic
  containers, and old food product lying around throughout the facility. These areas would include the restroom area, pop
  storage area, back storage rooms, and the drivethru area. Remove all the items and keep facility clean.
   
  N)      Maintaining premises - unnecessary items and litter.
   
       The premises shall be free of:
       
        (1) Items that are unnecessary to the operation or maintenance of the food service operation or retail food
       establishment such as equipment that is nonfunctional or no longer used; and
   
       (2)      Litter.

  VIOLATION Progress has made in this area on the wall cleaning.(Still some cleaning required) The register area walls ( pop
  and snack area) are not smooth and cleanable. Refinish / repaint walls in this area and make wall surface smooth and
  cleanable.
   
  (B)      Cleaning - frequency and restrictions.
   
       (1)      The physical facilities shall be cleaned as often as   necessary to keep them clean.
   
        (2) Cleaning shall be done during periods when the least amount of food is exposed such as after closing. This
       requirement does not apply to cleaning that is necessary due to a spill or other accident.
